  • Providing appropriate and safe care to Clients.  

The Health Care Aide will report directly to the HCA Supervisor and interacts with Clients, family and employees with a positive and supportive manner.

PROFESSIONAL AND PERSONAL QUALITIES 

  • Certificate of an approved accredited Health Care Aide program. 
  • Registration with the Alberta Health Care Aide Directory
  • Class 4 Driver’s License and clean Driver’s Abstract
  • Other certifications/competencies:
    • CPR certificate. 
    • First Aid certificate. 
    • Safe Food Handling certificate.
  • Related health care experience.

SUMMARY OF WORK:  

Under the direction of the Stabilization Unit Team Lead and as a member of the multi-disciplinary team, provide support and assistance to the Shelter and Stabilization Unit Teams.

PROFESSIONAL AND PERSONAL QUALITIES 

  • Assist with medical assessments when requested on Stabilization and Shelter. 
  • Cover floor areas for breaks and if staffing requirements are needed. 
  • Assist with pharmacy orders, MAR transcription and checking.
  • Take Vital Signs and complete medical assessments within the LPN scope of practice. 
  • Complete medical procedures such as wound dressing changes and assessments as needed. 
  • Liaison with health and service providers to Client/Guest(s) to, and give medical updates on care provided to Client/Guest(s) within the Shelter and Stabilization Unit.  
  • Ensure documentation is completed as per BTDH Inc., Policies and Procedures. 
  • Give report on Client/Guest(s) as needed to health and service providers as per BTDH Inc., Policies and procedures.  
  • Assist NP and medical providers with procedures and understanding LWSSU processes and procedures. 
  • Complete treatment medicals for guests with assistance from NP. 
  • Provide assistance to HCA as a regulated provider. 
  • Patient Treatments and Dressing changes per LPN scope of practice
  • Administer medications and injections per LPN scope of Practice, Clinic Policy and Nursing Assessments, and Documentation is completed.
  • May assist with clinical appointments and outreach under the direction of the NP. 
  • May do clinical education as instructed by NP. Ie. Hand Hygiene education. 
  • Ensure supplies are stocked and ordered as needed. Notify Coordinator is supplies need to be ordered. 
  • Weekly inventory and ordering of supplies and monitor for expired supplies. 
  • Specific ordering and supplies – go through supervisor
  • Other duties and tasks BTDH Inc., or Coordinator requires within the scope of LPN

SUMMARY OF WORK:  

The Health Care Aide is responsible for providing essential and important daily living support to Stabilization clients by providing appropriate and safe care as well as personal assistance and support; and encouraging independence and self-care.  The Health Care Aide is also responsible for providing support to the interdisciplinary team and provides respectful and culturally-safe services in a safe and supportive environment. This position may include working days, evenings and nights.

PROFESSIONAL AND PERSONAL QUALITIES 

  • Performs personal care for Stabilization clients on a case-by-case basis (i.e., pediculosis assessment). 
  • Records accurate and complete information in the Client record following the charting forms and policies of Blood Tribe Department of Health, Inc. and other legal requirements. 
  • Observes and notes any changes to Clients’ condition/status.
  • Provides skilled care to Clients according to practice standards by Blood Tribe Department of Health, Inc. policies and procedures.
  • Assist Clients with the provision of personal care:
  • Medication assistance as per MAP guidelines and policies under a regulated health care provider.
  • Hygiene and grooming (i.e., Pediculosis assessment). 
  • Skin care
  • Apply non-prescription lotion/creams
  • Apply prescription lotions/creams based on Supervisor’s approval.
  • May apply simple non-medicated dressings to cover a wound until further assessment can be conducted by a regulated health care provider. 
  • May assist with simple dressings or complete simple dressings under the direction of a regulated health care provider as per regulated provider recommendations. 
  • Communicates and reports updated and current Client status to Stabilization Unit Coordinator and Clinician.
  • Provides support for nurses and other health care providers (NP, Pharmacy, Nurse, Physician) with:
  • Cleaning equipment (BP cuff, BGM monitor, medical work areas)
  • Taking and recording vital signs and assisting with simple wound care under the direction or supervision of a regulated health care provider (Nurse, Nurse Practitioner, Pharmacist, Physician). 
  • Reports concerns for safety risks and hazards to Stabilization Coordinator and Clinician.
  • Observes Client condition and report changes.
  • Utilizes critical thinking, problem solving and decision-making skills.
  • Applies proper body mechanic principles
  • Able to sit, walk, transfer, lift, carry equipment and supplies.
  • Able to transfer guests (one/two-person, mechanical lift)
  • Able to position guests (bed, chair, wheelchair, etc.)
  • Cooperates and participates in joint projects.
  • Promotes a health/safe environment for the Clients, families and employees.
  • Confers with and shares information with multidisciplinary health care team to ensure continuum of care.
  • Works within scope of practice for Medication Assistance Program (MAP) guidelines and regulations. 
  • Participate in the process that will seek Clients and family input and feedback with respect to quality services.
  • Performs cleaning tasks as required, including sweeping and mopping, sanitizing commonly touched areas, gathering garbage and garbage-removal, and gathering towels, clothing, etc.
  • Performs other duties as requested, including but not limited to assisting on the Shelter side, completing wellness intake forms if wellness monitor not available, or leading programming for Stabilization Clients or Shelter Guests, if short staffed
  • Stabilization staff are consistently faced with complex situations; therefore, Health Care Aides are required to troubleshoot where needed and/or consult with the Stabilization Coordinator and Clinician if unable to do so. This also requires incident reports if the situation involves overdose and/or any involvement with emergency services. This must be done in accordance with Lethbridge Wellness Shelter and Stabilization Centre Policies & Procedures.
  • Acts with integrity and treats all information regarding guests, staff, visitors, service providers and financial and operational status of the organization as confidential.
  • Participates in the maintenance of a quality improvement environment by adhering to:
    1. Blood Tribe Department of Health Inc quality improvement initiatives.
    2. Blood Tribe Department of Health Inc. policies and procedures
  • Greets all individuals including Clients, visitors, service providers and employees in a professional manner respectfully.

SUMMARY OF WORK:  

The Health Care Aide is responsible for providing essential and important daily living support to Stabilization clients by providing appropriate and safe care as well as personal assistance and support; and encouraging independence and self-care.  The Health Care Aide is also responsible for providing support to the interdisciplinary team and provides respectful and culturally-safe services in a safe and supportive environment. This position may include working days, evenings and nights.

PROFESSIONAL AND PERSONAL QUALITIES 

  • Performs personal care for Stabilization clients on a case-by-case basis (i.e., pediculosis assessment). 
  • Records accurate and complete information in the Client record following the charting forms and policies of Blood Tribe Department of Health, Inc. and other legal requirements. 
  • Observes and notes any changes to Clients’ condition/status.
  • Provides skilled care to Clients according to practice standards by Blood Tribe Department of Health, Inc. policies and procedures.
  • Assist Clients with the provision of personal care:
  • Medication assistance as per MAP guidelines and policies under a regulated health care provider.
  • Hygiene and grooming (i.e., Pediculosis assessment). 
  • Skin care
  • Apply non-prescription lotion/creams
  • Apply prescription lotions/creams based on Supervisor’s approval.
  • May apply simple non-medicated dressings to cover a wound until further assessment can be conducted by a regulated health care provider. 
  • May assist with simple dressings or complete simple dressings under the direction of a regulated health care provider as per regulated provider recommendations. 
  • Communicates and reports updated and current Client status to Stabilization Unit Coordinator and Clinician.
  • Provides support for nurses and other health care providers (NP, Pharmacy, Nurse, Physician) with:
  • Cleaning equipment (BP cuff, BGM monitor, medical work areas)
  • Taking and recording vital signs and assisting with simple wound care under the direction or supervision of a regulated health care provider (Nurse, Nurse Practitioner, Pharmacist, Physician). 
  • Reports concerns for safety risks and hazards to Stabilization Coordinator and Clinician.
  • Observes Client condition and report changes.
  • Utilizes critical thinking, problem solving and decision-making skills.
  • Applies proper body mechanic principles

  • Participate in the process that will seek Clients and family input and feedback with respect to quality services.
  • Performs cleaning tasks as required, including sweeping and mopping, sanitizing commonly touched areas, gathering garbage and garbage-removal, and gathering towels, clothing, etc.
  • Performs other duties as requested, including but not limited to assisting on the Shelter side, completing wellness intake forms if wellness monitor not available, or leading programming for Stabilization Clients or Shelter Guests, if short staffed
  • Stabilization staff are consistently faced with complex situations; therefore, Health Care Aides are required to troubleshoot where needed and/or consult with the Stabilization Coordinator and Clinician if unable to do so. This also requires incident reports if the situation involves overdose and/or any involvement with emergency services. This must be done in accordance with Lethbridge Wellness Shelter and Stabilization Centre Policies & Procedures.
  • Acts with integrity and treats all information regarding guests, staff, visitors, service providers and financial and operational status of the organization as confidential.
  • Participates in the maintenance of a quality improvement environment by adhering to:
    1. Blood Tribe Department of Health Inc quality improvement initiatives.

Under the direction of the Fire Chief or designate of the Blood Tribe Emergency Services, you are accountable to deliver fire suppression, prevention, respond to dangerous incidents, and medical calls as First Responders.
